A useful plan should still make sense after the appointment ends. These worksheets give you somewhere to keep the questions, patterns and practical details that are easy to lose between visits.

Looking for a diagnosis
Start with the symptom timeline and appointment agenda (pages 2–3). Record when episodes happen, what changes with them and what life is like between episodes. Bring actual test reports rather than only a note saying a result was positive.

Living with a condition
Daily-impact notes (page 4) help you describe sleep, energy and activities. The school/work worksheet (page 5) focuses on support you need, without requiring you to share your entire medical history.
The care summary and monitoring plan (pages 6–7) can travel with you to another clinician. Ask your team to fill in the monitoring schedule and who will contact you about results.
Starting treatment or moving care
Use the treatment conversation sheet (page 8) to clarify goals, safety, training and access. The travel/refill/transition checklist (page 9) helps you plan before medicines run out or you change clinicians.
Complete the illness/flare plan (page 10) with your treating team. A blank worksheet is not a prescription or a set of treatment instructions.
Which diary should I use?
For someone still being evaluated, the general timeline is a starting point. For established FMF, CAPS, TRAPS or MKD, your team may recommend the separate AIDAI diary. The workbook is not AIDAI and does not create a disease-activity score. Keep daily-impact notes separate from that score.
Keep it useful and private
- Use short notes and dates; a concise summary is easier to discuss than dozens of disconnected screenshots.
- Update your medicines and contacts whenever they change.
- Download or print the workbook. This page does not accept completed worksheets.
- Do not wait to finish a form before seeking care for new or concerning symptoms.
